The superbill

This is the piece of paper. Here it is.

We don't bill your insurance. We hand you an itemised superbill and you submit it against your out-of-network benefit. People nod at that sentence and have no idea what they're agreeing to, so rather than describe it, here is the actual document — real codes, real charges, for every kind of visit we do.

Nothing here is sent anywhere or saved. Pick a visit and the document below changes. What your own plan does with it depends on your out-of-network deductible and coinsurance — the estimator works that part out.

Which visit?

You don't have to file it alone.

Every superbill leaves here complete — the diagnosis linked to the procedure, our NPI and tax ID, place of service, the lot. Nothing missing, nothing for an insurer to bounce it back over.

Most plans take it three ways: a photo uploaded in your insurer's app, a PDF through their member portal, or an app like Reimbursify that files out-of-network claims for a few dollars each and tracks them until they pay. Whichever you pick, we'll walk you through it the first time.

We don't submit on your behalf, deliberately — the moment we transmit a claim electronically we become something different under HIPAA, and staying out of that is part of how this practice works. It stays your claim, filed by you, paid to you.
