Everything on this website is general information about our practice. It is not medical advice, it is not a diagnosis, and it is not a substitute for a consultation. Reading it does not create a physician–patient relationship between you and Dr. Lloyd or thrē Clinic. That relationship begins only when we have accepted you as a patient and you have completed our intake.
Using the form
Submitting the form on our Begin page is a request for a conversation. It does not create a membership, reserve a place, or guarantee that we can accept you. Please do not include medical details — a web form is not a secure channel.
Prices
The prices published on this site are current at the date below and apply to memberships and services beginning in Bentonville, Arkansas. They may change. If you are a member, any change to your rate will be communicated to you directly and in advance. A founding rate is held for thirty-six months from the start of your membership, provided that membership is continuous, and then moves to the standard rate then in effect.
Fees for laboratory work, imaging, pathology, pharmacy and hospital services are set by those providers, not by us.
Insurance
thrē does not bill insurance. We provide an itemised superbill you may submit to your insurer under your out-of-network benefit. We cannot promise that any insurer will reimburse any amount, and we do not guarantee HSA or FSA eligibility. Amounts paid for identifiable medical services are ordinarily qualified medical expenses; a periodic membership fee may not be, and IRS guidance in this area changed in December 2025. This is not tax advice — your accountant is the right person to confirm your position.
