What you need, what it would cost, and what your insurance would likely do with the superbill. Honestly, including when the answer is "not much."

We're cash-pay, so we don't bill your insurance — but you get an itemised superbill for every visit and can submit it against your out-of-network benefit. The founding rate is $300 a month and it's held for three years; after the fifty are taken, membership is $400 and the practice closes at 400. We're not going to pretend that's urgent — but it is finite, and we'd rather tell you the number than imply one.
